| The Medicare Coordinated Care
Demonstration Project has awarded a grant to Quality Oncology, Inc. to offer
a care management program to Broward County, Florida cancer patients with Medicare
coverage. Experienced cancer nurses will educate and support patients and their families,
and facilitate communication between patients, their physicians and other health care
providers. As patients learn to better manage and coordinate their health care needs, they
may face fewer emergency room visits and hospitalizations, and enjoy an improved quality
of life. Quality Oncology (QO) is the oldest and most experienced cancer
care management company in the country, providing services for over 12,000 patients. We
would like to offer you the same high quality service that we provide to these patients.
You will have telephone access to an oncology nurse, at no cost to you.
Your care manager will be a registered nurse who has specialized in taking care of cancer
patients. This care manager will:
- Help you understand your disease and treatment options,
- Help you with your symptom management after treatment,
- Help you prepare questions before you go to the doctor,
- Help you access any support services that you may need, and
- Provide any other emotional or social support that you may need.
The only site selected by Medicare to address the unique needs of the
cancer patient, Quality Oncology expects to enroll 2000 eligible and
interested patients. Ideally, patients will enroll at the time the cancer
diagnosis is made, prior to beginning treatment. Those who have recently
begun cancer treatment will also be considered, but the program is not
intended for patients who have completed treatment or are in remission.

to refer a newly diagnosed cancer patient. Participation is
voluntary, with no cost to the patient. Patients are free to
disenroll at any time, so there is no obligation and no risk
involved.
WHAT ELSE YOU SHOULD KNOW
It is important to understand that because this is a demonstration project, the
government is seeking to assess the value of our care manager service. Patients will be
randomly assigned to our service or to a control group of patients, who do not have the
benefit of our services. This means that if you choose to participate in this
demonstration program, you will be randomized and possibly not be assigned a care manager. |

Am I Eligible?
YES if you are enrolled in Medicare Part A
and Part B and you have a valid Medicare number.
YES if Medicare is your primary insurance.
YES if you live in Broward County.
YES if your primary Oncologist practices in
Broward County.
YES if your cancer has been confirmed through a
biopsy.
YES if you are just beginning treatment for
cancer or cancer recurrence.

What Would Make Me Ineligible?
- If you have already completed active treatment for your cancer or are in
remission.
- If you are enrolled in an HMOs Medicare + Choice Program.
- If you are enrolled in hospice.
- If you are a member of a United Mine Workers or Railroad retirement program.
- If you have been diagnosed with basal or squamous cell skin cancers.
- If you have been diagnosed with pre-cancerous conditions of the cervix, prostate
(dysplasia), bladder or colon.
- If you are a patient taking maintenance hormonal therapies (Lupron, Zoladex,
Tamoxifen, Megace.)
- If you have been diagnosed with advanced Renal Disease.
